STARTING THE OMMP CARD PROCESS

Our clinic will need a copy of the patient's most recent medical records that document his or her qualifying medical condition .

If you have the records, you can bring them to our office, fax them to 541-582-9150, or email them to info@rrherbal.com

.    If you need to get records, you can come to our office and fill out a Medical Release form and we can obtain the records from your doctors office for you.

ONCE THE RECORDS ARE RECEIVED

They are reviewed by our Dr. Helen Trew.  When approved, we will contact the patient to schedule a clinic appointment where the patient will meet with our doctor and our knowledgeable staff will provide the patient with all the information and forms needed to obtain their Oregon Medical Marijuana Program card.    

WHAT TO BRING TO YOUR APPOINTMENT

In an effort to make the process as simple as possible for our patients, we ask that they bring their government issued photo ID (driver's license for example) and any proof they have of a discount allowed by the state.
